An exciting opportunity has arisen within the Macmillan Information & Support Service at Harrogate District Hospital for a Cancer Information and Support Adviser. We are looking for an enthusiastic, knowledgeable, highly motivated and organised individual with excellent communication skills to support the Macmillan Information & Support service which will be based in within the Sir Robert Ogden Macmillan Centre

You will be joining an established and expert Cancer Information & Support Team and will be responsible for offering expert patient information and support for patients’ practical, emotional and social needs. You will be leading and facilitating Patient Education Programmes and undertaking holistic needs assessments to agree personalised care and support plans with patients that address the concerns of those living with and beyond cancer including their families, friends and carers. There will be significant opportunities to develop and you will be supported to do this with a clear development plan. Participation in the provision of clinical supervision and undertaking service evaluation and audit will be paramount.

The post holder will manage the delivery of comprehensive cancer information, education and support service within Harrogate and District Foundation Trust for people affected by cancer including patients, carers, family and friends as well as health and social care professionals.

The post holder will ensure access to high quality information and compassionate support for all service users that is accessible via multiple formats and mediums of delivery and link with other agencies locally, regionally and nationally. They will also work closely with all clinical teams to share expertise that enhances the experience of those affected by cancer.

The post holder will support projects to improve patient experience, gain service user engagement and support clinical teams in the continued implementation of the personalised care agenda and the agreed stratified cancer pathways for follow up.

Communicate sensitively and compassionately in often very emotional situations with patients, families, carers who may be very distressed

Demonstrate specialist skills to develop and undertake health and wellbeing events to promote the principles of patient self-management in consultation with the information and support manager and the clinical teams. These will include generic patient education programmes at the beginning, during and end of treatment and some clinically specific events.

Actively promote and participate in the implementation of key principles of the Personalised Care agenda in line with the NHS long term plan

Support the manager in the continued development of an Information and support strategy in line with the Trust values and strategy working in partnership with key stakeholders

Develop and provide high quality information for use across multiple media formats, e.g. social media, website and video.

Participate in the ongoing development of the service so that it remains patient focused and meets the needs of the local community
