An exciting opportunity for the vacancy of Lead Commissioner - Learning Disabilities and Autism has arisen, following the successful promotion of our current lead. The post holder will be the lead commissioner to oversee strategic commissioning of health, social care linking with education services specifically for all children, young people and adults with a learning disability, including those who display, or are at risk of developing, behaviour that challenges. The post holder will ensure the Integrated Care Board in conjunction with its Transforming Care Programme partners has developed Learning Disabilities & Autism Spectrum Disorder (ASD) strategic plans and will ensure the Transforming Care Programme (TCP) has joined-up commissioning arrangements. The holder will have in-depth knowledge and experience of working with children, young people and adults with a learning disability and behaviour that challenges, including knowledge of local service

The post holder will ensure that patients’ voices are heard and that patients’ and carers’ views are used to improve the quality of patient care and develop community services that meet patient need.

The post holder will provide senior leadership and support for the Transforming Care programme for people with Learning Disabilities and Autism Spectrum Disorder (ASD). This will entail working with a range of stakeholders including providers, specialised commissioning and the local authority to ensure that community services are developed and established to facilitate the discharge of patients from hospital and prevent admission. The post holder will also liaise and work with neighbouring Transforming Care Programme (TCP) to ensure that plans for building support in the community are delivered.

The post holder will be required to undertake On Call duties on a rota basis

**Key Result Areas**:
To provide senior leadership to ensure the delivery of the Transforming Care plan, working with relevant stakeholders and establishing programme work streams as required for the delivery of the programme work plan.

To work in partnership with Birmingham City Council Learning Disabilities Joint Commissioning Team, Solihull Metropolitan Borough Council Commissioners, NHSE Specialised Commissioning, Joint Children’s Commissioners and Mental Health Commissioners to ensure effective arrangements for the discharge of patients from secure units into the community.

To ensure that there are effective arrangements in place for Care and Treatment Reviews and Care, Education and Treatment reviews and that these are effective in informing discharge plans and prevent future admissions.

To ensure that appropriate community services are established to enable patients to be discharged from hospital and that these services meet the complex and individual needs of patients.

To work with commissioners are Transforming Care Partner’s in the West Midlands in developing commissioning plans in terms of future bed provision and community support. To liaise, where appropriate, with Transforming Care Partners colleagues across the Midlands region to share best practice, local intelligence and benchmarking information to support delivery of the programme.

To ensure governance arrangements for the Transforming Care Programme are effective, providing leadership and support to the Transforming Care Programme Board, Transforming Care Programme Advisory Group and Transforming Care Partners Sub Groups and including regular reporting to the relevant Integrated Care Board and Council Committees ensuring compliance with Information Governance requirements.

To ensure that clients and carers/families are at the heart of the Transformation Plan and that they are involved in co-production of services and support.

To provide leadership in the development of personalised care including processes for joint health and social care funded Personal Budgets, Education Health and Care Plans and Integrated Personalised Commissioning.

To ensure there is an integrated approach to patient safety, performance management, quality and the continuous improvement of commissioned services, challenging providers accordingly and undertaking themed reviews and investigations into quality issues as appropriate.
