Care Coordinator

Care Coordinator role available for 24hpw working either 4 or 5 days a week.
**Main Purpose**:
- To provide coordinated care for all registered patients’ resident in care homes.
- To provide coordinated care for all registered housebound patients.
- To maintain patient electronic records for clerical reasons, the Quality and Outcomes Framework and for medical care. The records must be accurate and contemporaneous.
**Key Tasks**
2. Proactively contact all the care homes at least weekly to discuss patient related concerns.
3. Navigate the non-urgent requests and the patient related concerns to the most appropriate service or clinician
4. Contribute towards the creation and maintenance of the advanced care plans (including when appropriate frailty score, GSF level, date of last SMR, etc).
5. Ensure all information about the patient is updated - using appropriate ARDENs templates and associated read codes.
6. Support alignment of care home to practices, including registration, updating care plans and liaising with GP team to ensure medications are updated.
7. Maintain and update records of all housebound and patients in care homes who have been admitted to secondary care (date admission discharge, reason for admission, post discharge contact.
8. Coordinate MDT meetings with the practice GP and community team.
9. Manage and prioritise workload on a daily basis and dealing with the competing demands of the care coordinator role.
**Other Admin tasks**
- Helping to administer COVID vaccination data whilst the COVID vaccination programme continues.
- Attend regular team meetings.
- Take part in any training as required from time to time.
- Work to the schedule as set by the line manager and report any backlogs of work.
- Any other duties considered relevant to the post.

The post-holder should recognise the importance of effective communication within the team and will strive to:

- Communicate effectively with patients and carers.
- Recognise people’s needs for alternative methods of communication and respond accordingly.
Office hours between the hours of 8:00am - 4:00pm Monday - Friday
