The CYP Complex Care team is a division of the HIOW All Age Continuing Care service (AACC), working closely across a life course pathway with the adult Learning Disability Continuing Healthcare (CHC), Placements (HCP) teams, ICS, Section 117 aftercare, Learning Disabilities and Autism Programme, the CAMHS Provider Collaborative for Wessex & Dorset (Sussex Partnership Foundation NHS Trust) and Local Authority partners.

**The role is hosted by the Hampshire and Isle of Wight Integrated Care Board (ICB). Children with Learning Disabilities and Autism (LDA) with complex needs are a key priority, with teams across an all-age pathway working together to improve services.**
