Care Coordinator Needed for Clinical Care Service

- Salary: £25,000 per year, depending on experience
- Must Drive and be willing to travel distances around the North Midlands
- Work from Home & Visiting patients in their own homes - Children/Adults Clinical Care patients, Learning Disability, Autism and Mental health,

- Ability to recruit staff to care packages and manage rotas
- Willingness to cover emergency shifts (12 hour) as and when needed

Driving is essential as all clients are community based.

**OUR LOCATIONS**:
Our patients are based in Birmingham, Staffordshire, Tamworth, Sandwell, Willenhall and surrounding areas.

**REQUIREMENTS**:

- Possess the motivation and initiative to work in a fast-paced growing company
- Experience of clients with complex clinical care needs
- Experience of clients with Learning Disability/Autism care needs
- Excellent oral and written communication skills

- Work in partnership with nurses / healthcare staff

**FUNCTION OF ROLE**:

- You will be responsible for managing staff, client care packages, working alongside an experienced Care manager / Nursing team
- Setting up new packages of care and new staffing of services
- Shadow new care workers in the field to ensure competence and quality
- Carry out on-site observation and evaluation of care worker practice as part of routine supervisions, manage performance concerns, manage HR issues under direction of a managers
- Follow accident and incident reporting procedures.
- Support carers to deliver. excellent standards of care
- Carrying out assessments, support planning, reviews and investigations
- Monitor changes in client health, behaviours and circumstances and respond appropriately when this change.
- Be part of Rota for Out of Hours On-Call (evenings / weekends)
- Ensure services delivered by iCare Living meet CQC legal standards
- Use own initiative and work alone and as part of a team especially in an emergency
- Work in partnership with service users, their family, carers, advocates, other healthcare professionals
- Monitor quality of records and work to constantly improve quality of output
- Meet monthly Key Performance Indicators of the role

**KEY DUTIES**
- Assessing new clients and completing care plan documentation and risk assessments
- Undertake regular client reviews and update client care plans / risk assessments
- Help with recruitment including shadowing and mentoring new employees
- Carry out spot checks and follow up performance reviews with care staff
- Complete and undertake regular service audits raising any concerns to the Registered Manager
- Support the Registered Manager with office duties and administration
- Provision of hands-on care where required
- Cover shift for staff absences
- Work as part of the team to ensure our systems of work remain compliant with CQC / CCG’s, local authority and commissioners.
- Participate in staff training and team meetings
- Complete staff supervisions
