We are looking to recruit a Care Navigator to the newly developed Autism and Learning Disabilities Liaison Team within CAMHS Oxfordshire.

We have a very exciting opportunity and are looking to appoint one of the first Care Navigators working with 18-25 year old's in the newly developed Autism & Learning Disability Liaison Team for CAMHS Oxford.

We are looking for an innovative professional who will work collaboratively with key multi-agency partners, children/young people (CYP) and their families.

This role will be particularly attractive for someone who enjoys working collaboratively with other agencies to make sure we ‘get it right’ for children, young people and their families.

You will be a system changer who has a real passion for making a difference to CYP, their families and carers in a person-centered way. This will involve day-to-day co-ordination and influencing commissioners, healthcare staff and wider professionals. Therefore a high level of negotiation and leadership skills will be essential.

You will work with CYP, up to the age of 25, with the most complex needs and their families and carers to make sure families are fully involved in their plans, feel listened to and informed, plans are personalised, and they have the support they need at the right time, in a co-ordinated way. Keyworking should help families experience a reduction in stress and uncertainty and an increase in stability.

The Care Navigator will utilise the Dynamic Support Register (DSR) and work closely within the Care Education and Treatment Review (CETR) process to identify the CYP most at-risk of admission to a mental health hospital. They will have oversight of their holistic programme of multi-disciplinary care and will proactively support the CYP to navigate, troubleshoot, challenge contentious issues and respond to escalating needs through their range of interventions and pathways.

This will be an exciting development opportunity as you will be part of the implementation of a new service. We have good structures of both Clinical and Managerial Supervision as well as Peer Supervision/Reflective Practice Groups.

- This role involves a high level of organisation and planning in partnership with multi agency partners and families. This will include having an influential voice in the planning, formulation and adjustment of complex care programmes agreed at CETR meetings (care education and treatment review). This will impact how services work together to support CYP who are at the highest risk of admission to a mental health hospital.
- Make crucial judgements that will influence how services support CYP who are at crisis stage and, therefore mitigation against risk of sexual and physical abuse, complex family issues, self-harm and suicide risk is an important consideration. This requires the ability to make good decisions quickly by using complex analysis, challenging contentious issues and selecting best options to reduce
- Appropriately manage and report safeguarding concerns and collaboratively work with Social Care and other multi-agencies to manage safety risks and keep CYP
- Work with health, education, social care and the third sector by providing and receiving complex, sensitive information and breakdown barriers to understanding in order to achieve a set of key outcomes based on what matters to the
- Work in partnership with the Thames Valley Provider Collaborative and Local Authority colleagues to ensure smooth facilitation of transition into and out of secure settings, and into adult services for young
