The role is to ensure seamless and effective care of the patient by working between the medical and ward nursing teams, troubleshooting problems and escalating to the senior surgical team when required. They will act as a highly specialised role model and leader for all members of staff and play a major role in the continuous development of the services within the Trust and as a potential member of a range of Trust quality/process improvement project groups. The role involves working approximately 2 night shifts per 4 week roster, in the capacity of the night practitioner role. Whereby you will lead the Hospital at Night Team (HAN) across both the Lung and Heart Divisions, including the evening and morning medical handover processes. The role will include cross-covering for other advanced clinical practitioner roles within the cardiothoracic services unit.

The post holder will form part of the Heart Division cardiac surgical team, working as an Advanced Clinical Practitioner (ACP). The post holder will share their previous skills and experience in the field of cardiac surgery.

The job is practically based, intended to ensure skills and knowledge are related to high quality care and efficient delivery of service. The post holder will also undertake projects at the discretion of the Care group.

**Responsibilities**:

- Patient/customer care (both direct & indirect)
- Work autonomously as an advanced practitioner within the Heart Division, delivering individualised direct patient care.
- Use best evidence, advanced specialist skills, knowledge and expertise to provide expert clinical practice
- To integrate into the existing clinical structure and to support other professionals to manage the peri-operative period for surgical patients.
- The ACP will undertake routine management of the surgical patient including regular monitoring of peri-operative patients, communicating with the ward staff where clinical concerns arise and take responsibility for ensuring the discharge process is efficient in relation to decision making; discharge letters and ordering of take-home medication.
- The ACP will undertake history taking, clinical examination, arranging relevant investigations (e.g., blood tests, radiographs and scans).
- This will include both patients who attend pre-admission clinic and those admitted on to the wards.
- To lead the Hospital at Night Team (HAN) across both the Lung and Heart Divisions, including both the evening and morning medical handover processes.
- To be the first point of contact for all ward nursing teams across the divisions and to triage clinical activity to the relevant person within the HAN team.
- Receive referrals from the wards directly, providing expert initial assessment to any patient meeting the calling criteria for the CCO team or HAN service and provide night time review of all patients in respiratory medicine.
- Provide night time review of all patients in Respiratory Medicine areas. To liaise with the Respiratory Medicine MDT, HDU MDT and AICU MDT to facilitate timely admission and discharge of patients.
- Undertake Non-medical prescribing (having undertaken the NMP course, and passed and been identified and signed as competent, and following approval of the trusts NMP and medicines management boards)
- Ensure through good practice that customer care initiatives are improved and sustained. Act as a resource for expert advice to patients, relatives/carers and staff.
