**Position**: **Care Coordinator for the following areas: Thetford, Attleborough and Wymondham**

**Location**:Snetterton Branch

**Pay Rate**: £23,000.00

**Job Type: Full time, permanent position.**
- Ensure service users receive their arranged visits at a time that is appropriate to the individual wherever practical
- Allocate visits appropriately to care workers, whilst also providing support to care workers, service users and/or families
- Demonstrate a working knowledge of a Care Planner Roster
- Manage and co-ordinate multiple projects
- To hold the on-call phone at least one week a month
- Demonstrate excellent IT and communication skills both written and verbal
- Develop good and effective relationships both internally and externally
- Demonstrate an ability to ensure confidentiality
- Work as part of a team and ability to problem solve
- Excellent attendance and time keeping record
**Providing care for vulnerable people in their own homes**:

- You will assist our service users to meet their care needs, which can range from personal care to social interaction.
- You will encourage our service users to do things for themselves, wherever possible, working in a person-centred way.
- You will make a positive difference to people’s lives by helping the most vulnerable within your community.
- You will be able to observe the progress that our service users make every day and be part of the outcomes.
- Salary
- Signing bonus £150
- Care rate from £11.50 up to £12.50
- Free DBS check
- Contract
- 28 days paid annual leave (pro rata)

- Paid In-house training provided (Via Zoom App) which includes Care Certificate and ongoing training.
- Recommend a friend scheme (£150 ‘Pending completion of 3 days Training’).
- Care Coordinator or Quality Assurance experience gained within Care Industry (required).
- Excellent communication skills, and ability to remain calm under pressure.
- NVQ/QCF level 3 diploma or working towards (required)
- Free DBS check
- You will have to be a Caring, Trustworthy, Professional, Friendly person and to able to work well with others with dignity and respect.
