Macmillan Cancer Support Worker in Head

Embark on a fulfilling role as a Cancer Support Worker, where you'll be a cornerstone of support for Head & Neck cancer patients throughout their diagnostic and treatment journey. This is a unique opportunity to contribute meaningfully to the lives of those facing challenging health circumstances.
You will support Head & Neck cancer patients throughout their journey from diagnosis to completion of treatment. Holistic Needs Assessments will play a central part of this role which allows the Cancer Support Worker (CSW) to plan a supportive care plan that will aid patients through their diagnosis and treatment. The CSW plays a key role in ensuring patients receive the right care at the right time for the right reason - a significant part of this aspect of the role will include ensuring co-ordination of the multidisciplinary head and neck Pre & Post Treatment Clinics.
Effective communication is key, as you collaborate with surgeons, oncologists, and Allied Health Professionals, working under the supervision of the esteemed Clinical Nurse Specialist team.
In addition to your support responsibilities, you'll work closely with the Patient Navigator in Head & Neck and provide cross-cover during periods of professional and annual leave. This role is embedded within the reputable Macmillan Head & Neck Clinical Nurse Specialist team, renowned for excellence in the Lancashire & South Cumbria region.
In this dynamic role, you will be the central point of contact for all non-routine inquiries, ensuring smooth communication and information flows by initiating and responding to correspondence. Your crucial responsibilities include skilfully resolving inquiries for an optimal experience.
You'll play a pivotal role in delivering a personalised and supportive holistic needs assessment service for cancer patients. This involves coordinating access to valuable information and educational resources, empowering individuals to make informed decisions about their care.
Effective communication is at the core of your responsibilities as you liaise with colleagues, patients, and cancer services. This ensures a well-coordinated and patient-centric approach to care.
Handling incoming calls, you'll use a risk assessment framework to triage effectively. Collaborating with the Clinical Nurse Specialist (CNS) in non-routine situations, you'll refer complex decisions to the team for assessment and review.
Additionally, you'll provide basic telephone advice, expertly referring or signposting individuals to other sources of support, and take charge of coordinating patient questionnaires to collect valuable data and feedback.

- Contribute to the assessment, implementation and evaluation of care to meet patients’ needs in accordance with agreed local and national standards and evidence based guidelines.
- Understand that there will be frequent exposure to distressing/highly distressing situations and when to seek appropriate support/advice
- Support and provide patient education and health promotion
- Communicate all relevant information to the multidisciplinary team internally and with external organisations in a timely and professional manner, to ensure seamless care
- Alongside the CNS team, develop and maintain the service Information leaflet.
- Support patients with cancer and their families to access appropriate information and support, by sign-posting to a range of clinical and support services
- Co-ordinate and track patient pathways patients with cancer.
- Arrange and book appropriate outpatient appointments.
- Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit and outcome data.
