It’s an exciting time to join us in Adult Social Services. Using the ‘Living Well’ approach to care our vision is to promote independence and support recovery for people with mental health needs.

We are looking to recruit an enthusiastic and hardworking Assistant Practitioner to join the South Mental Health social care team. Based at Gateway House and within Adult Social Services, you will support existing staff to review and assess service users using the Care Act 2014. You will work closely with our key partner, Norfolk and Suffolk Foundation Trust where you will provide an integrated approach to assessment and care along with the Norfolk Integrated Housing & Community Support Service (NIHCSS).

Working in a friendly, supportive team you will assess and review care and support needs, enabling people to work towards greater independence through creative support planning. You will be passionate about making a difference and ready to embrace change. A team player, you will need to demonstrate an ability to work under pressure, be well organised and able to effectively prioritise. Ideally you will have experience of working with adults with complex needs and have either experience/interest in working within the mental health field of social care.

You will have experience, knowledge and commitment to strength-based assessments. The ability to travel is essential.
