**About us**

We are looking to recruit to the post of Nursing Associate, to work within our Primary Care Network multidisciplinary healthcare team.

The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging environment is essential.

The Nursing Associate will work with the Registered Nurse and other members of the multi professional team, providing and monitoring the condition and health needs of patients within their care, contributing to the ongoing assessment, as agreed with supervising clinician(s) and in line with the service pathways. You will be expected to support patients in the management of long term conditions (Asthma, Diabetes and COPD), Cervical Cytology, Family planning, Child Health/Immunisations, Wound Management, Travel Immunisations and all other general treatment room duties.

They will provide first point of contact care for patients presenting with undifferentiated, undiagnosed problems utilising history-taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and their carers where applicable).

You will be working as part of a multi-disciplinary team to monitor care under direct or indirect supervision, using a wide range of skills you have including (but not limited to);
compliment the role of the GP and other Health Professionals

make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients.

provide a safe, high quality and accessible treatment room service to patients.

As the Nurse Associate you will act within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care under supervision

You will show safe clinical decision-making and expert care for patients within the general practice, in consultation with the duty GP. You will work with the multi-disciplinary general practice team to support the delivery of policy and procedures and meet the needs of patients.

Supervision and mentoring will be provided by the appropriate senior medical staff - the type of support will depend on the post holders skills and knowledge and determined by our clinical governance arrangements.

**Salary**: £27,000.00-£31,000.00 per year
