Are you a motivated and experienced mental health practitioner (OT/RMN/SW)?
- We are looking for enthusiastic, compassionate, & experienced mental health practitioners to join our well-established, friendly, supportive Community Mental Health Team for Older People (CMHTOP).
- Working closely with patients and families/carers, there will be a particular focus on delivering high intensity time limited community interventions aimed at providing alternatives to mental health acute/crisis admissions and to facilitate early discharge, reablement and mental health recovery.
- This involves supporting core team tasks, telephone triage & support, signposting, face to face assessment, care planning and referrals to relevant services.
- These positions require a high level of autonomous practice, practitioners will be confident clinicians able to make competent decisions about risk, care planning and appropriateness of home treatment. Postholders will have relevant experience of community mental health acute/crisis work.
- The MDT team includes nursing, OT, psychology, support and medical staff. All team members are valued and encouraged to participate in CPD & career progression.
- Postholders will be supported by regular supervision, applicants from a range of professions, including nursing, OT and social work are welcome.
- To provide a clinical service that is service user focussed.
- To work within a multi-disciplinary team providing an effective professional and high-quality community service
- To be responsible for specialist assessment, provision and evaluation of planned/unplanned clinical interventions and care of older people with mental health problems, encouraging and developing family and community support whenever possible and appropriate.
- To use a person-centred, compassionate and evidenced based approach to working with people with complex mental health needs and who may also have cognitive and physical health needs.
- To develop individualised care and therapy plans for patient's experiencing a deterioration or crisis in mental health, including working with other health and social care services/staff to provide advice/input as necessary.
- To provide intensive home treatment in home/community settings
- To provide or facilitate crisis management and short term biomedical, psychosocial and other MDT mental health interventions/ therapies as required (this may include medical, nursing, occupational therapy, psychological, physical health or social work interventions/treatments as indicated)
