**Job Details**

We have developed a unique exciting position which we believe Suffolk County Council is the first Local Authority to do so. This professional advisor will sit between Social Work Services West Suffolk and Home First (Adult Social Care in house reablement service). Leading the local integrated team develop practice and advice on what does a high-quality short-term assessment look like and feel like to our customers.

The role will benefit from both sitting with Social Work Services and supporting front line social work practice as well as being a part of our ‘Outstanding’ Home First service, the largest CQC registered home care provider in Suffolk for this new and exciting role. Helping us achieve our ambitions of having all those that require it access to high quality reablement services. This is part of Home First ambitious development and expansion project.

**Your role and responsibilities**

The primary purpose of the role is to lead on behalf of Home First and the wider Adult Social Care department the development of Short term assessments for customers. This is where a customer maybe new to the service or have new care and support needs and will have a short term reablement and assessment period. We are committed to our aim of all short term assessment periods to be of an outstanding quality where the customer is empowered to regain their independence.

You will be working as part of the Social Work local management team as well as the Home First management team. You will work direct to Operational and partnership Social Work managers with a dotted line to the Deputy Head of Home First, supporting the Home First and Reablement Services expansion project.

**You will need**
- relevant professional Social Work, Occupational Therapy or similar health or social care qualification to degree level, or equivalent, with evidence of continuous professional development
- HCPC or relevant other registration
- experiencing in social work practice and supporting others with their practice
- understanding of short term assessments and the relevant legislation
- the ability to support cultural change between different teams and find mutual objectives.

**The team**

You will be apart of the local Social Work Service and Home First teams. Below is a paragraph on each:
**Social Work Services** in Adult and Community Services (ACS) works with adults over the age of 18 providing professional Social Work and Occupational Therapy advice and carries out assessments and reviews based on a Signs of Safety approach, involving the Adult, their family and wider network. Our Social Work teams are made up of Social Workers, Occupational Therapists and Independence and Wellbeing Practitioners. Each team has its own Team Manager who in turn is supported by an experienced senior management team. Team Managers are supported and managed by the Operations and Partnership Manager

We are a key stakeholder in the wider Health and Social Care integration within Suffolk and have joint roles and responsibilities at a local level with Social Work Services and NHS colleagues.
