**A bit about the role**

Powys Community Additional Needs Project (PCAN) provides support to children and young people aged 0-19 with additional needs and/or a disability. We assist children and young people with disabilities on an individual basis, either to access activities or groups in their own communities, or to carry out activities in their own homes.

As a Young Persons Assistant you will be responsible for providing support to Children, Young People and Families by agreeing and directly delivering interventions to enable positive outcomes to be achieved.

- Organising and providing outreach support to children and young people.
- Empowering children and young people to thrive within their community.
- Adapting to the needs of the children, young people and their families.

**Let's talk about you**
- NVQ 2 in Health and Social Care.
- Experience of supporting children and young people with disabilities and their families.
- Knowledge of child protection processes, safeguarding and confidentiality
- Flexibility to work evenings and weekends depending on the need of the service.
- Registration or willingness to register with Social Care Wales, which we will support you with
- Full clean driving licence and access to own vehicle including business insurance. The job requires the ability to travel according to the needs of the job, with reasonable adjustments, if required, according to the Equality Act.
- Welsh language (desirablenot essential)
