An exciting opportunity has arisen for a temporary, part time Social Care Practitioner to join the Amber Valley North Prevention and Personalisation Team.

We work with adults who have care and support needs (and people approaching adulthood) living in the community and in other settings to complete person-centred assessments.

An understanding of the Care Act 2014 principles and Strength Based Models of practice is essential.

The role involves visiting people at home, or in other placements to gather information, seek individual aspirations, positively influence their wellbeing and maximise opportunities to promote choice and control.

Safeguarding enquiries are also part of the role where strong communication and organisational skills are essential.

Working with our NHS colleagues and other partner organisations are a major aspect of the role therefore excellent communication skills are key to secure more successful outcomes for people we support.

A hybrid working arrangement is adopted therefore working from both the office and from home is required.

We pride ourselves in having a good supportive team who welcome the opportunity of sharing their knowledge and skills with both experienced and new, non experienced colleagues.

A robust induction, as well as regular monthly supervisions and team meetings are provided in order to help develop practice and experience.
