Overview:
Aplos Health PCN was founded in 2019 and consists four practices in the London Borough of Lewisham -Sydenham Green Group Practice, The Vale Medical Centre, Wells Park Practice and Woolstone Medical Centre. We provide care to a patient population of 50,000. In a little over a year and despite a global pandemic we have developed excellent working relationships between the practices. We have run educational events for patients, started an adolescent and contraceptive clinic and are now looking forward to developing further innovative pathways and services for our patients.
We are seeking an experienced, self-motivated, hard-working, and patient focused healthcare assistant to join our PCN. They will work closely with the GPs and other primary care professionals within the PCN to proactively identify and manage a caseload of patients, making sure that holistic support and is made available to them and that their complex needs are addressed. They may also be asked to contribute towards other administrate tasks within the GP Practice or PCN.
**Job Purpose**:
Care coordinators provide extra time, capacity and expertise to support patients. They work closely with the GPs and other primary care professionals within the PCN to proactively identify and manage a caseload of patients, making sure that holistic support and is made available to them and that their complex needs are addressed. They may also be asked to contribute towards other administrate tasks within the GP Practice or PCN.
The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.
To assist in the development and implementation of change to improve the service provision in the PCN
To work collaboratively with colleagues across health, social care and the third sector to provide patient-centred, integrated care.
**Main duties of the job**:
This is a split role between IT support and Care Coordinating
PCN IT support and facilitation for all PCN staff and projects including;
- Create and maintain appointment session templates and weekly planner
- Create and support EMIS user access
- Create and support SMART card access
- Responding to general IT enquires
- Provide EMIS inductions to new starts
- Monitor dashboards for PCN performance against DES targets
- Complete ad hoc data analysis and investigations requests from PCN Manager
- Build and run EMIS reports
- Facilitate the data collection for digital inclusion projects and competent in IT skills to support the implementation of new software and telephony systems across the PCN
Care Coordinator duties include;
- Proactively manages patient call and recall system for those with complex care needs.
- Responds to all queries and requests for assistance from patients within care target group in regard to their care plan and all aspects of their clinical management. Signpost patients and their carers to appropriate support both inside and outside the practice.
- Monitors delivery of care by completing patient meetings; documenting care; identifying progress toward desired care outcomes.
- Reviews the care plan with patients interacting with involved departments to negotiate and expedite care where possible.
- Works with clinicians to ensure all the contractual requirements for patients with complex care needs are met.
- Manages referrals and provides information/ advice for a range of different community healthcare services.
- Assists in the administration and coordination of MDT meetings, ensuring team recommendations are completed in a timely manner.
The role also involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.

**Clinical System / Medical Records**:
- Confidence and experienced user in -EMIS
- Extraction of necessary data from clinical correspondence and input into Electronic Patient Record, i.e., GP2GP and Docman system
Ensure full and accurate recording in the patient’s computerised records.
**Safeguarding**:
The post holder will be expected to undertake safeguarding training (Children and Adults) appropriate to their role and adhere to policies and procedures relevant to the area they work in.
