We are seeking a Specialised Mental Health Case Manager with mental health clinical and practice background to be part of the For Me (Thames Valley and Wessex) Adult Secure Mental Health Provider Collaborative.

Oxford Health NHS Foundation Trust (OHFT) is the Lead Provider for the For Me (Thames Valley and Wessex) Adult Secure Provider Collaborative which brings together inpatient and community providers to improve clinical care across the Thames Valley and Wessex region to ensure that patients receive inpatient treatment as close to their home as possible. This is an innovative network of partners across a large geographical footprint covering Oxfordshire, Buckinghamshire, Berkshire, Milton Keynes, Hampshire, Isle of Wight and Dorset.

The Provider Collaborative brings together clinical expertise, experience, and innovation, aiming to improve quality, use resources most effectively, and deliver best practice consistently to all patients.

The post holder will actively be working in partnership to innovate to continuously improve service quality and patient outcomes.

The Case Manager will provide clinical support to the Provider Collaborative in relation to the defined population which will include projected mapping of future service use and planning in terms of future service needs.

The Case Manager will work as part of a Case Management Team supporting managers and the multi-disciplinary team to ensure safe and effective care pathways are in place for all service users. They will work across the Collaborative to provide quality oversight to ensure commissioned services meet the required standards.

The postholder will work closely with partners to embed the principles of prevention, early intervention, community-based provision and least restrictive practice. They will maintain effective, open and clear communication with Providers, NHSE, commissioners and Local Authorities to ensure strong joint working.

Office space is available in across the network services. The role will involve travel both within and outside of the Provider Collaborative footprint.
