Lincolnshire County Council has an exciting opportunity in for **Lead Practitioners within our Hospital Service**. This is an excellent opportunity to work within the hospital discharge and transfer of care service.

**About the Role**

The role is for a **Lead Practitioner** to manage the Adult Care team within an Acute Hospital environment, within **Pilgrim Hospital, Boston**.

You will be **supporting** and **encouraging** your team to have the confidence to work in a strengths based way.

The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we support are at the **very centre** of their own discharge. We think **creatively** when planning support packages, utilising “**outside of the box**” thinking and solutions to ensure an **individual** care plan is in place that **empowers** the service user to be an active voice and participant. Our highest continuing aim is to provide **the best service possible** to service users and their families.

LCC’s Hospital Service is leading the way on the introduction of a fully integrated Discharge to Assess Hub, to support access to the right services in a more fluid way, reducing delays and delivering benefits to service users.

**You will work as part of the 7-day Adult Care rota. This currently equates to approximately 1 in 5 weekends worked, and these are all scheduled 3 months in advance.**

If breakfasts are more your thing, then our team office is also the place to be as our self-proclaimed ‘Fat-Free Fridays’ are a joy to indulge in at the end of the week

**About You**

**It goes without saying that you must be a professional Social Worker, Nurse or Occupational Therapist with significant experience to undertake this role.**
