**About The Role**

Are you passionate about making a positive impact in the lives of older people? Are you skilled in connecting individuals with the support and services they need? We have an exciting opportunity for a dedicated and compassionate individual to join our team as a Community Agent focused on reducing social isolation among older people. As a Community Agent, your primary responsibility will be to provide assistance, support, and guidance to older individuals, helping them overcome social isolation. You will serve as a central point of contact, understanding each individual's unique needs and working collaboratively with local groups, professionals, and services to provide comprehensive support and referral pathways.

Hybrid working is offered with a mixture of office, out in the community and homeworking and some flexibility over days worked can be considered.

**What you will need to be successful in the role**
- Serve as the main referral point for older individuals seeking support to reduce social isolation.
- Accept referrals from external sources, such as family members, community nurses, bereavement support services, and other organizations working with older people.
- Connect older individuals with physical and social activities, both internally and through external services, to reduce social isolation.
- Build meaningful relationships with older individuals to understand their needs and break down barriers to social engagement.
- Develop a thorough understanding of local provisions for older people and share information about social events and activities.
- Proactively identify and engage socially isolated older individuals, offering regular communications, visits, practical assistance, and referrals.
- Collaborate with volunteers to enhance and expand the scope of the service.
- Work harmoniously with colleagues from other services to deliver a cohesive and holistic support approach.
- Strong commitment to supporting older individuals and reducing social isolation.
- Have excellent interpersonal and communication skills to connect with older people and collaborate with diverse stakeholders.
- Knowledge of local community resources and services for older individuals.
- Willingness to adapt and learn, staying informed about innovative solutions for reducing social isolation.
- Ability to work independently and as part of a team to achieve organizational goals.
