**Ref: S1080 | Home-based, Waltham Forest, London, however, regular travel will be required as part of this role (will include team meetings or other work related meetings) | 21 hours per week**:
**This is a fixed-term maternity contract until November 2024. Our services are contracted, we currently have funding for this contract until 31 March 2024.**

We’re looking for an enthusiastic and motivated individual to join our Waltham Forest Team in London, reporting to the Service Delivery Coach.

**Key responsibilities will include**:

- Organise and facilitate effective service communication support group activity for the benefit of stroke survivors and carers in understanding and achieving their goals.
- Ensure that timely, confidential and accurate records are kept on our CRM data base and all data is in line with our retention policy and GDPR compliant.
- Develop and manage the service volunteer to support service delivery for stroke survivors and carers as required.

**You will have experience in**:

- Experience of organising and facilitating effective support group activities.
- Experience of providing person centred support to empower vulnerable people or people with a disability or long term health condition and their carers.
- Ability to nurture emotional resilience needed to handle a variety of calls, potentially dealing with complex and challenging situations whilst working in your own home.
