**Job overview**:
The post holder will provide a pivotal role in enabling safe and timely discharge, through active discharge planning from the point of admission to reduce delayed transfers of care.
Effective discharge planning provides a multidisciplinary approach to continuity of care; a process that includes identification, assessment, goal setting, planning, implementation, coordination, and evaluation. Effective discharge planning supports the continuity of care, providing a critical link between treatment received in hospital by the patient, and post-discharge care provided in the community.
The post holder will be a health or social care professional with expertise of working within Mental Health services.
This post sits within the Patient Flow team, this incorporates bed management and discharge coordination.
The wards are spread over two hospital sites, Bodmin Hospital and Longreach House.

**Main duties of the job**:
To lead the co-ordination of service user care in complex aftercare cases; discharge or transfer as appropriate, ensuring both health and social care needs are fully assessed. The post holder will need to be able to bring together stakeholders, with differing viewpoints and priorities, to reach consensus on individual discharge plans. Communicate and liaise with service users and relatives/carers, health service staff (hospital and community), Social Services and appropriate agencies and providers, providing sensitive patient information.
The post holder will work as an integral member of the patient flow team, although based on the wards. They will supervise patient flow coordinators.
