- Lincolnshire Post-Covid Rehabilitation Service

For some people recovering from COVID-19 brings longer term challenges to their physical and mental health once their acute illness is over. We are bringing together existing services across the Lincolnshire Health and Care system to help people navigate and access the support they need for rehabilitation and recovery. We are looking for a motivated and dynamic rehabilitation assistant that can help to deliver specialist care plans to patients in the community.

This is an exciting opportunity to work closely with a wide range of professionals such as occupational therapists, physiotherapists, dietitians, nurses, consultants and mental health practitioners to develop and deliver this new service. There will be a strong emphasis on using technological solutions to support self-management, goal setting and symptom tracking.

The role will involve supporting with the flow of patients through the service such as triage of referrals.

The service will be delivered countywide, primarily utilising digital solutions such as e-consultations and also in a range of venues, including patient’s homes. The post will require the ability to travel to meet the needs of this county-wide service.

Experience of working with people who have physical or mental health needs is essential, but you will be working under the supervision of registered professionals who will provide the training and development you need to deliver the care plans they devise. You will also be supported by the Post-Covid Rehabilitation lead.

This role will involve supporting of the day to day running of the Lincolnshire Post Covid Service, and will require input within our weekly virtual MDT.

Main duties will include the completion of virtual screening calls, review calls and triaging of new referrals of patients, under the direct supervision of the care coordinator. There will also be a requirement to support with virtual rehabilitation groups and Peer support groups the service offers. This role will also include supporting with day to day administration tasks to allow for a smooth flow through the service. Although some face to face patient contact may be required within this role, the majority of the work will be carried out virtually.
