Assistant Practitioner Nurse

An exciting opportunity has arisen for an Assistant Practitioner Nurse to work within the Long Term Care Team.
The post holder will be responsible for reviewing patient’s eligibility for NHS Funded Nursing Care and NHS Continuing Health Care in care homes and community settings, where appropriate, in line with The National Framework for Continuing NHS Healthcare Wales 2021.
The post holder will support the nurse assessor with coordinating and undertake multiagency disciplinary team meetings to establish an individual’s eligibility for NHS Continuing Health Care. Ensuring effective multi-disciplinary and partnership working in relation to following process.
The post holder will work closely with Nurse Assessor and colleagues from Neath Port Talbot and Swansea Local Authority undertaking Joint Monitoring Visits of providers from the Independent Private Care Home Sector.
The post holder will be part of a dynamic, friendly and enthusiastic multidisciplinary and multiagency team.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Referring individuals for multi-agency disciplinary team assessments whose physical and mental health care needs have changed following discussions with nurse assessors.
To report any concerns related to patient care following review to the Nurse Assessor/Team Leader within the Long Term Care Team.
The post holder will work independently within established policies and procedures and provide care as delegated by a registered healthcare practitioner.
