The post holder will provide advice and expertise on the management of adults who are survivors of childhood cancer and will provide the nursing lead for long term follow up care. The post holder will be the keyworker for those coping with the late effects of disease and treatment. The post holder will provide and maintain the essential link between the children's and young persons service. This will include working autonomously and managing a caseload within the long term follow up service. The role is operational, clinical and strategic, working in close collaboration with multi-professional teams locally and nationally, to ensure the development of best practice in the care of adult survivors of childhood cancer. The education and career development of the post holder will be supported by following the late effects career and competency framework

We are looking for a motivated Registered Nurse (Adult) working at a senior level to be a keyworker to support patients who have been treated for childhood cancers. The post holder will provide advice and expertise on the management of a range of conditions. You will need to be able to work independently and link with clinical teams across the Trust to provide the best possible care for patients with a range of complex health needs.

KEY JOB RESPONSIBILITIES

Clinical

Utilising a highly developed specific body of knowledge, works within a clinical team to deliver a service based on principles that are research based and which enhance the quality of patient care.

Assess and interpret patients / clients with undifferentiated or unstable problems within scope of practice.

Undertakes high level complex decision making, managing risk but may defer to others for overall plan.

Be a visible point of contact for patients, visitors, relatives, and staff, acting as the patients advocate

Ensure that patient’ records and documentation are maintained and current, with changes implemented, in accordance with NUH ‘Clinical record keeping policy’.

Organise meetings to discuss patient management as

Provide highly specialist support to clinical teams in dealing with the complex needs of different client groups.

Liaise closely with other health and social care professionals to ensure a coordinated approach and high quality care is provided for patients.

Discuss alternative treatment options and facilitate clinical management decisions.

Obtain informed consent for procedures as required, in accordance with the Trust consent policy.

Participate in research studies in conjunction with the specialty team and lead studies as appropriate.

Promote and support personalised care in line with the NHS long term plan including the components of the comprehensive model for personalised care.

Provide clinical support to wards/departments in times of increased clinical pressure or staffing shortages as requested, ensuring that patient safety is the priority at all times.

Education, Learning & Development

Undertakes, and can assess those undertaking, any ‘Working in New Ways Packages’, which have been identified as integral to the role requirements and specialty area.

Provide a credible source of highly specialist clinical knowledge and advice, taking an active role in the development of self and others. Review the performance of self and others, identifying development needs and taking active measures to meet those needs.

Create a positive learning environment within clinical settings, to encourage and facilitate individual and shared learning and professional development.

Participate, in the education of patients and the general public, developing resources as necessary.

Maintain an awareness and understanding of relevant, up to date research studies within the clinical field.

Supports and leads as required, on the development of WINW packages related to the individuals highly developed specific body of knowledge.

Leadership and Management

Support the senior team as appropriate.

Respond to and lead as necessary on clinical governance initiatives eg complaints, datix

Initiate and implement change effectively in response to clinical or organisational demands. React positively to, lead and facilitate the process of effective change through a culture of Shared Governance and frontline leadership.

Be a representative at a local level and participate as a member of the senior nursing team/ Speciality, making positive contributions to
