**We currently have an exciting opportunity within the Medicines Management Team for a full-time Care Co-ordinator (Patients on Specific Medication). This would suit an organised, enthusiastic and methodical administrator. This is a new role attached to Trowbridge Primary Care Network, working across both practices.**

Main duties of the job

The role will involve working closely with the Clinical Pharmacists and the Medicines Management Team - to identify and organise blood tests for patients who are using specific medication.

Trowbridge Primary Care Network covers the Trowbridge area and a population of almost 50,000. There is a strong team spirit throughout the organisation. Both practices enjoy working together to produce the most efficient services for our patients within our town.
