The purpose of this role is to provide high quality support to older people recently discharged from hospital. You will have a clear understanding of the needs of older people and will actively promote their wellbeing during their resettlement at home and as they can regain their confidence to once again manage their daily activities.

Support will be person centred and will focus on preventing readmission to hospital. You will actively promote the wellbeing of older people and will provide practical and emotional support to allow them to remain living independently. You will also provide
advice and guidance and signpost to other relevant services as required to facilitate the needs of older people.

You will manage your work time and case load effectively, have a passion for supporting older people and be flexible in order to meet service needs.

The ability to drive and have access to of your own vehicle for work purposes is essential to your role. You will also undergo an enhanced DBS check
