Care Coordinator

Job Summary:
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for assisting in the coordination of care services for clients and ensuring their needs are met. This is a vital role in our organization as you will be the main point of contact for clients and their families.
**Responsibilities**:
- Coordinate and schedule care services for clients, ensuring coverage and continuity of care
- Conduct client assessments to determine their needs and develop personalized care plans
- Collaborate with healthcare professionals, caregivers, and family members to ensure effective communication and coordination of care
- Monitor client progress and make necessary adjustments to care plans as needed
- Provide guidance and support to caregivers, answering any questions or concerns they may have
- Maintain accurate and up-to-date client records in compliance with privacy regulations
- Assist with the recruitment and onboarding of caregivers, conducting interviews and background checks as required
- Participate in team meetings and training sessions to stay updated on best practices in care coordination
**Skills**:
- Excellent organizational skills with the ability to prioritize tasks effectively
- Strong communication skills, both verbal and written
- Proficient in using office software such as Microsoft Office Suite (Word, Excel, Outlook)
- Ability to work well under pressure and handle multiple tasks simultaneously
- Compassionate and empathetic nature, with a genuine desire to help others
- Attention to detail and accuracy in documentation
