An opportunity has arisen for a passionate and self-motivated individual to join our team as a **First Point of Contact Assessor** based in **Swansea**.

**Hours**:18.75 per week

**Salary**:£10,753.88 per annum

**Holidays**:25 days per year plus 8 public holidays (pro rota)

**Responsible to**:Treatment Services Team Leader

**Job Purpose**:
This is an exciting opportunity for anyone who is looking to work within substance misuse. The post holder will be responsible for assessing substance users aged 18 and over across the Western Bay area. Duties will include carrying out assessments including risk, developing care plans with individuals and provide brief interventions and make onward referrals to the appropriate agency as well as keeping accurate records.

**Responsibilities**:

- To carry out all other tasks commensurate with the role.
- Undertake comprehensive substance use assessments, risk assessments, and develop care plans to meet identified needs.
- Deliver brief interventions.
- Conduct screening phone calls for substance users wanting to access drug and alcohol services.
- Monitor the Newid referral inbox and contact newly referred clients.
- Identify appropriate services to meet identified needs.
- Work in partnership with service users to reduce alcohol and drug related harm.
- Develop close and collaborative relationships with a wide range of service providers, both in statutory and voluntary sectors.
- Provide support to those waiting to be allocated to a service
- Attend relevant meetings to update partnership agencies of the work being completed.
- Maintain appropriate written and database client records and undertake appropriate administrative tasks in a timely manner. The WCCIS database is currently being utilised.
- Participate in team meetings and regular supervision sessions.
- Maintain confidentiality in accordance with Adferiad’ s policy and procedure, and with any statutory/contractual requirements.
- Support individuals who may attend the agency in crisis
- Work in a non-judgmental, anti-discriminatory way at all times.
- Promote a positive profile of all local substance misuse treatment agencies.
- Organise and manage time effectively to ensure service delivery opportunities are maximised.
- Take responsibility for updating training needs and knowledge requirements such as relevant legislation, and practice/research about substance misuse.
- Carry out all tasks with professionalism and in line with the agencies policies and procedures.
- Promote a positive profile of the agency.
- Undertake other reasonable duties as requested by the management team.

**Qualifications -**

NVQ Level 3 in Health and Social Care, or equivalent relevant professional qualification or be willing to work towards

**Experience-**
- Experience of delivering services to substance users.
- Experience of carrying out assessments and developing care plans.
- Experience of liaising with other specialist and non-specialist agencies.

**Skills -**
- Substance use assessment and care planning skills.
- Ability to complete risk assessments and manage risk.
- Excellent communication and interpersonal skills.
- Good IT, report writing and note keeping skills, including maintaining all client records as required.
- Ability to organise own time and work on own initiative.
- An ability to work as part of a team.
- High level of motivation and enthusiasm.

**Knowledge -**
- Knowledge of local and national resources. Knowledge of different approaches to working with substance use.
- Knowledge of legislation pertinent to the role including Safeguarding.
