Main responsibilities To be responsible for assessing, delivering and reviewing high standards of holistic, individualised care, support and symptom management to patients and their families in their own home or the IPU at the end of life in accordance with Isabel Hospice working practices. To monitor the quality of care, promoting high standards in accordance with the policies of Isabel Hospice and the Care Quality Commission standards. Provide information, advice and support to patients, families and carers face to face and via the advice line and to be aware of escalation procedures. Demonstrate interpersonal skills that promote clarity, compassion, empathy, respect and trust.

Advise patients in relation to their illness and their families and carers on their specific needs and to enable expression of preferences and wishes in relation to future care in a safe environment. Assess and manage symptom control with support from the Multidisciplinary Team. Pre-empting preventable, physical symptoms and reducing psychological distress. Maintain and promote effective and efficient working and communication with other members of the multi disciplinary team across community and hospitals, care homes, voluntary private and statutory agencies.

To actively promote high standards of care in all settings. Promote privacy, dignity and respect of all patients, their families/carers and all staff. Provide, implement and evaluate individualised care recorded in the care plan and when required participate in multi professional meetings, acting as patient advocate and representing aspects of the patients management. Understand psychological issues as well as family dynamics affecting terminally ill patients and their families, providing support and advice specific to their needs.

To participate in audits, as required. Act as a role model to junior staff to support their development. Promote evidence based practice and assist with implementation of changes to the service as required. Personal responsibilities The post holder will: Maintain professional and courteous working relationships with all staff and those with whom contact is made, respecting diversity and ensuring quality at all times.

Be responsible for taking reasonable care with regard to self as well as for colleagues, patients or visitors who might be affected by any act or omission by the post holder. Act as an excellent role model by upholding and implementing good practice in the workplace. Quality Participate in monitoring standards of care, policies and procedures through audit and patient satisfaction monitoring. Work as a team to ensure there is compliance with the requirements of the CQC and other regulatory bodies.

Be a champion for patient safety, clinical effectiveness, and patient experience. Ensure all patient related information is treated sensitively adhering to Isabel Hospice policy. Report any accidents or incidents and raise any concerns as per Isabel Hospice policy. Ensure ability to manage electronic record system taking into consideration the need to protect access (ID, passwords, swipe cards).

Maintain a professional appearance in line with Isabel Hospice policies. Uphold Isabel Hospices values & behaviours and behave in a manner fitting to the position of the post holder. Undertake any other duties which reasonably fall within the scope of the role. Professional, Training & Development Work to the NMC/HCPC code to prioritise people, practice effectively, preserve safety and promote professionalism and trust.

Take personal responsibility for lifelong learning and development using clinical supervision, appraisal and actively engaging with learning and development opportunities. Attend mandatory training to comply with statutory and regulatory requirements of the NMC/HCPC and to work in accordance with Isabel Hospice policies and procedures.
