This role is being implemented to ensure a safe and timely discharge of all our patients on the 56 bedded unit.

The GI Unit is comprised of two busy surgical wards Page & Northumberland totalling 56 inpatient beds. Our specialist patients have undergone extensive GI surgery and so have complex discharges weekly. A key aspect of this role will be effective communication with our on site and community colleagues to ensure safe and supported discharged for all our patients.
- To risk assess and identify complex discharges on admission, and in collaboration with the multidisciplinary team set discharge dates to assist the Trust in meeting it’s NHS access targets.
- To work in collaboration with a dedicated multidisciplinary team, Social Services and other relevant personnel in both the hospital and community to plan and manage timely and appropriate transfers of care (discharge) for patients from the hospital.
- To work across Page and Northumberland Wards on a daily basis, to negotiate and organise transfer of care arrangements for patients
- Act as an advocate for patients discharge, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines.
- Ensure that patients’ receive high quality clinical care and have a positive patient experience, having regard for their customs, religious beliefs and doctrines.
- Attend and participate in multi-professional team meetings to facilitate safe discharges.
