The Nelson Medical Practice is a modern practice based in Wimbledon Chase and has an exciting opportunity for a Patient Care Navigator to join our team. This is a great opportunity to work in an innovative and fast expanding GP Practice in Merton. We are passionate about providing high quality patient centered care to our 30,000 patients. We have 2 roles with varied hours ranging from 2 shifts per week to 5 shifts per week ( 9 hours per week and 30 hours) per week from Monday - Friday. Once trained in this role you will be expected to work 4 Saturday mornings per year.

Good communication and customer skills are essential as this role involves dealing with members of the public both on the telephone and face to face, good IT skills are also essential.

Job Summary:

- To assist the Dr's and other Clinicians in the delivery of high quality service
- Receiving and directing patients on arrival at the Practice in a courteous and efficient manner
- Answering telephone calls coming into the Practice and dealing as appropriate
- Giving results to patients
- registering new patients onto the Clinical system
- Booking appointments
- attending relevant meetings and staff training
- Answering general queries and explaining processes

Person Specification:

- Good IT skills
- Good communication skills
- Good organisational skills
- Ability to multi - task
- Ability to work to deadlines

**Job Types**: Part-time, Permanent

**Salary**: £10.67 per hour

- Overtime
