The trust provides specialist forensic mental health care across eight boroughs in North West London, in addition to national enhanced medium and high secure services.

This exciting role sits within the Clinical Governance structure and will work in collaboration with the Patient Safety and Complaints Lead and Clinical Director.

In addition, the post holder will work with clinicians, service users and carers and will be integral to the development of high standard investigation reports and implementation of subsequent plans.

You will be responsible for the collection and analysis of complex information in order to produce high quality investigation reports and for developing and supporting the implementation of subsequent plans. You will provide regular reports and presentations to the service governance meetings.

You will work with clinicians, service users and carers to develop robust and meaningful investigation reports.

Adopting a person centred approach, you will engage with those affected by serious incidents to ensure they are able to participate in the investigation processes. This will include speaking with service users and carers who wish to raise any concerns over aspects of care and treatment delivered by the Trust.

This role will encompass working across all areas of the service, offering the opportunity to develop effective joint working with clinical and non-clinical staff from both within and external to the Trust.

You will be both undertaking and supporting complaints investigations.

The post holder will be responsible to the Patient safety and Complaints Lead, WLFS and accountable to the Clinical Director, WLFS.

**Undertaking reviews and investigations**:

- To undertake investigations and reviews into Complaints.
- Maintain a database of active investigations and reviews, prioritising and structuring your activity to most effectively meet demand and reporting
- Facilitate patient safety review meetings with staff groups and other appropriately interested
- Complete complaints investigations within specified and agreed
- Complete complaints investigation reports to a high standard using the agreed reporting
- Maintain a database of outcomes and action plans from SUI and complaints investigations and to upload this information to Trust systems.
- To monitor completion of action plans
- To identify and report any problems or deficits
- To ensure complaints/SUIs are allocated appropriately for investigating, and that the staff support component of the complaint/SUI process is also allocated and
- To work alongside a range of staff from across the service line undertaking investigations, in order to provide support and guidance to these staff as required.
- To monitor and ensure high quality complaint reports and response letters are produced, ensuring these provide the information required in a sensitive and clear
- To quality assure the way investigations are conducted and resolved, including providing direct feedback and suggestions for improvement to the investigation
- To facilitate the service in meeting its obligation in relation to a duty of candour.
- To respond to requests for information from legal representatives, the CQC and other external agencies, as required, on behalf of the service.

**Facilitation of learning into practice**:

- In collaboration with Patient Safety lead, Clinical Director and service or team managers, support the implementation of agreed aspects of the work plan.
- Share specific learning and actions at relevant trust-wide and WLFS forums.
- Track the learning and implementation of improvements as part of a cycle of continuous quality improvement.
- Contribute to other relevant Trust-wide forums and initiatives as a way of sharing and disseminating lessons that improve patient safety - e.g.: training events, Suicide Prevention Group, clinical audit work plans.

**Governance**:

- To provide the SMT with the outcomes and trends relating to complaints / investigations and incidents through
