Over the year long post, Our Care Connector will work with 80 people in the Harrogate and Rural District for a maximum of 12 weeks, and provide a personal assessment for people living with severe frailty. The personal assessment will include referring and signposting to other organisations, creating a full working multi-disciplinary team to identify what those organisations are, including where and how to access them. To receive referrals for anyone with a Rockwood score of 6 or over, and support that person with, and review of, the social, physical, wellbeing and practical issues within the home or life of the person. Key Tasks To provide personalised support to individuals, their families and carers to enable people to take control of their health and wellbeing, live independently and promote their health and social outcomes.

To match the individual with a wide range partner organisations including GP practices, pharmacies, hospitals, the fire and police services, jobcentres, social care services, housing associations and voluntary, community & social enterprise organisations that supports their needs, as well as internal referrals within the North Yorkshire Hospice Care Family of Services, including Volunteer Services, HOME service, Wellbeing Team, internal clinics and groups and the wider public, private and voluntary sector services. To develop trusting relationships by giving people time to focus on what matters to them. To co-produce a personalised health and wellbeing support plan, with clear, set goals introducing or reconnecting people to community groups and statutory services, with a focus on: Isolation and loneliness, Identifying driving services for appointments, Wellbeing, Looking at finances/resources available To manage and prioritise the service caseload, keeping accurate and contemporaneous notes on all interactions To recognise when it is appropriate or necessary to refer people back to other health professionals/agencies To take a lead on working with Volunteers to support the individuals identified To coordinate and support the delivery of Multi-Agency Meetings to facilitate individualised patient care plans. To organise and attend relevant meetings when required including Integrated Care meetings.

To confidently manage difficult conversations and support the individual with planning for the future, including wishes at end of life, thinking about finances, entitlements, funerals, memory work, working with the Well-Being Team.
