Lead Medical Examiner

North West Anglia NHS Foundation Trust (NWAFT) introduced a Medical Examiner (ME) system in 2020 as part of its Mortality Review Process. The Medical Examiner system will move to a statutory footing in 2024, with all deaths not referred directly to the coroner being scrutinised by a medical examiner.

The NWAFT Lead ME needs to be an appropriately trained, experienced Medical Examiner from a consultant or general practice background. Their role includes the following:

- Lead the team of medical examiners
- Chair monthly team meetings
- Identify learning from excellence
- Ensure independency of medical examiners
- Support medical examiners with difficult cases
- Audit performance of medical examiners and manage accordingly
- Carry out annual reviews of medical examiners to inform their appraisal
- Look for trends of poor care and escalate accordingly
- Escalate concerns accordingly
- Report regularly to the deputy medical director
- Present findings at the Mortality Surveillance Committee - quarterly
- Work closely with the bereavement team
- Work closely with the SJR team
- Ensure robust governance processes are in place to deliver an excellent service
- Attend regional medical examiner meetings
- Share relevant information with the medical examiner team
- Appoint new medical examiners and medical examiner officers

The NWAFT Lead ME leads the ME team, providing training and support to ensure service delivery.

Medical Examiners have professional independence in scrutinising deaths, but are accountable to the Medical Director and report to the Mortality Surveillance Committee and Regional Medical Examiner for achieving agreed standards.

Medical Examiners must avoid any potential conflicts of interest.

The principal responsibilities of the Lead ME, in addition to working as a medical examiner, are to:

- Develop and deliver the ME service
- Work with GP practices to ensure that community deaths are reviewed promptly
- Provide advice about how to formulate the cause of death and complete the Medical Certificate of Cause of Death (MCCD)
- Provide advice on the appropriate referral of cases to the coroner
- Liaise with the MEs to arrange cover for holidays and other periods of absence
- Ensure that there is no potential or perceived conflict of interest between the medical examiner and the death being scrutinised
- Support the training of doctors in completion of MCCDs and provide feedback on accuracy of certification locally
- Comply with local protocols to ensure that each death is screened in a way that is robust, proportionate and consistent.
- Take responsibility for own continuing professional development and take part in continuing medical education activities, in accordance with any relevant standards for maintaining GMC licence to practice and membership of any relevant professional body
- Be committed to the concept of lifelong learning and produce and maintain a Personal Development Plan in agreement with yourappraiser
- Attend relevant local and national activities in order to maintain up to date knowledge and to ensure compliance with legal and procedural requirements associated with the current processes of certification, investigation (by coroners) and registration
- Participate in any relevant governance activities relating to the scrutiny of deaths
- Chair regular meetings of the medical examiner team
- Communicate effectively with the Coroner and Registrar of Births and Deaths.
