We have an exciting opportunity for a highly motivated and enthusiastic Therapy Support Worker to join our team on Beech Ward, Norwich Community Hospital. Beech Ward provides specialist inpatient rehabilitation for people who have had a stroke.

This is a new role and you will work as part of the inter-disciplinary team providing assistance and support to the therapy team. You will assist in delivering individual therapy programmes and care to patients under the close guidance and supervision of Allied Health Professionals. We aim to support patients reach optimum function and independence.

This role can be Band 3 or linked grade providing an opportunity to progress from a Band 2 to Band 3 in line with an agreed set of competencies. We are looking for someone who has a basic understanding of health care and experience working in a team. You will be supported in your development, enhancing knowledge and skills in health care and stroke rehabilitation. *Band 3 competencies are expected to be reached with 1-2 years of appointment, with successful completion of competencies.

We are continually developing our Stroke Specialist Rehabilitation Services and you will be an integral part of this.

**The post-holder will be expected to**:

- have good interpersonal skills and the ability to speak empathetically with service users, relatives and carers.
- work as part of the multi-disciplinary team providing support to Allied Health Professionals carry out individual therapy programmes and care for patients
- work alongside and facilitate links with all members of the multi-disciplinary team
- meet the holistic needs of patients, including washing, dressing, continence needs, observations, food and fluid intake and wellbeing requirements
- manoeuvre wheelchairs, therapy devices and manual handling equipment such as hoists
- participate in routine data collection and administrative tasks as required to facilitate the effective management of both patient and service
- contribute and be involved in service development.

Beech Ward, Stroke Rehabilitation consists of 24 specialist rehabilitation inpatient beds and has a friendly and dynamic inter-disciplinary team. The ward is part of the Neuroscience/ Stroke pathway of care for Central Norfolk.

The Neuroscience/ Stroke pathway is developing with the aim of fostering further collaboration and partnership working across services. Norfolk Community Health and Care NHS Trust are reviewing models of care needed within our specialist rehabilitation services, linking to the Neuroscience/ Stroke pathway, to best support patients in our care. This is an opportunity for the post holder to be involved in these developments and explore the Therapy Support Worker role as these services and networks evolve.
- to work collaboratively with a variety of health, social care and professional staff.
- Perform straightforward, routine and specific delegated treatment programmes/ care for which you have achieved competencies. This will include supporting with therapy groups, community assessments and home visits.
- Work under the close guidance and supervision of Allied Health Professionals and organises own day to day tasks to ensure timely completion
- To work with patients towards agreed goals.
- Maintains service user equipment, including stock control, delivery and setting up.
- To accurately document all treatment and care delivered to patients, including recording documentation electronically on trust systems always maintaining confidentiality.
- Have an awareness of barriers to communication such as aphasia, cognitive impairment and mood. Uses a range of communication methods.
- Help induct and instruct new staff support staff
- To be flexible to the needs of the working environment
- Participate in service evaluation and quality improvement activities relevant to own work.
