The successful applicant will work alongside Dr Baker (7PAs), Dr Goggin (8.5 PAs) and Dr Bush (6.5PAs) to co-ordinate, support and develop the delivery of specialist palliative care by the multi-professional team across the care settings: in the hospice, the acute hospital and community. There can be flexibility with regard to the exact nature of the post depending on the successful applicant’s interests. There are also many opportunities to develop, or continue, a specialist interest and joint working is already well developed with the oncologists, neurologists and heart failure team. There is also the opportunity to become a Medical Examiner and support the acute trust in this respect. At present the palliative medicine consultants jointly provide the medical examiner role for 1 session a week. Full training and development would be provided through the Royal College of Pathologists.

**Clinical Duties**:
The consultant will deliver specialist palliative care to patients with life limiting illness and complex physical and/or psycho-social needs.

**Education and training**:
The post holder will contribute to the palliative care education program for hospital and community medical, nursing and social care staff.

**Service development / management**:
Participation in monthly senior team meetings and regular consultant meetings

Support acute trust and community initiatives to improve end of life care for the wider population

Audit and research

Contribute and lead local, regional and national audits as appropriate to the service.

Contribute to the team and Trust’s research agenda. The team are keen to develop our research input and are part of the Wessex Regional Research Network as well as collaborating with research initiatives across the BSW ICS

**Clinical Duties**:
The consultant will deliver specialist palliative care to patients with life limiting illness and complex physical and/or psycho-social needs.
- Act as a key senior member of the Palliative Care Team, liaising with the other Consultants, Senior Nurses, Team Leaders and other staff members, ensuring a multi-disciplinary approach to palliative care.
- Provide clinical support to specialist palliative care nurses, other clinical nurse specialists, allied healthcare professionals and other team members, leading change in practice related to supportive and palliative care across the Trust.
- Share clinical responsibility for the Hospice in-patients with the other Consultants. Patients are allocated geographically, with consultants having responsibility for specific catchment areas.
- Be responsible for overseeing the care of patients known to the Community Palliative Care team, dividing the catchment area with the other consultants.
- Provide Consultant Palliative Medicine input to the Hospital Palliative Care team.
- Develop close working relationships with colleagues in related specialities such as Medical and Clinical Oncology, Haematology, Cardiology, Respiratory, Neurology, Acute and Chronic Pain Teams and the site-specific multi-disciplinary teams.
- Promote close working relationships with Primary Care, providing clinical advice, undertaking domiciliary visits and Out-Patient assessments as appropriate.
- Work with healthcare professionals within the Trust and in the wider community, with external providers of palliative care (voluntary sector and social services) to co-ordinate and optimise the provision of supportive and palliative care.
- Ensure effective working relationships are fostered, based on good communication between professionals in the shared-care management of patients requiring palliative care.
- Promote good communication, information and integration of services for patients and their carers
