As part of the Futures Project within Adults Southampton we are looking to recruit two fully qualified Advanced Clinical Practitioners (ACP) to work within our Southampton Response services. These are brand new and exciting posts as we develop our ACPs workforce across Southampton. Both roles will involve working as an ACP team across the three localities with key working relationships with community nursing, Community Independence Service (CIS) who provide health and social care services including rehabilitation to the adult population of Southampton, and GP Practice and Primary Care Networks.

Working as part of the multi-profession team the roles will involve working across all four pillars of advance practice providing strong clinical leadership within the teams and helping to prevent hospital admission. This will involve assessing, diagnosing, creating management plans for complex patients, and having difficult and challenging conversations with patients and service users. The role will also include the element of long-term condition management and the completion of compressive geriatric assessments.

Supervision and ongoing development will be supported by Consultant Geriatricians and Consultant Nurse.
