**JOB DESCRIPTION**

**Job Title: Be Well Community Support Worker (Community Based)**

**Responsible to: Services Director**

**Hours: 35 hours per week**

**Main purpose of job**:
To provide well-being support, coaching and advice to older people accessing our community support service in ways that enable older people to achieve their goals and live their best life. Deliver community-based advice and support for people living with dementia and their carers living in Tameside.

**Core Role Responsibilities**:
Deliver one to one support sessions focusing on older people’s health and well-being

Deal with referrals and other service enquiries and complete client needs assessments and risk management plans.

Undertake goal setting support plan activities with clients eligible for community-based services.

To support clients through signposting to access local community services to support their wellbeing and enhance their quality of life.

Offer support and advice to people living with dementia and carers in the community.

To facilitate a weekly wellbeing group for carers and people living with dementia to support positive relationships and enable access to peer support.

To ensure all activities are appropriately risk assessed and any mitigating actions agreed by senior staff and implemented accordingly.

To provide escort support to clients to enable them to access centre activities, facilities and transport whilst attending the 131 Club.

To respond to and report any safeguarding concerns raised following the charities safeguarding policy, procedure and other support documentation.

To ensure all client data and sensitive information is handled and processed following the charities Information Governance, policy, procedure and other support documentation.

To ensure all client, staff and volunteer sensitive data, personal and commercial information is recorded and stored in designated hard copy files and computerised record systems.

To support clients to provide meaningful feedback about all the services they access and participate in service evaluation and surveys as part of improving the customer experience.

To work as part of a team to deliver all aspects of the service and to take an active role in personal and professional development.

To be able to demonstrate a clear understanding of service contractual target, outcomes and outputs and collect, collate, and report data to meet key performance indicators.

To be able to demonstrate a clear understanding of the charities values through your work.

CRITERIA

**Person Specification**

**Be Well Worker (Community Based)**

**Qualifications and Training**
- An NVQ 2 or above in Health and Social Care or other relevant qualification.

**Experience/ knowledge**
- A minimum of 2 years work experience with adults, older people in social care or the voluntary sector.
- Proven experience of person-centred working, asset-based approaches and/or a coaching model of support within a professional setting.
- Experience of responding to and recording safeguarding concerns
- An understanding of common issues affecting older people such as dementia, long term conditions, social isolation, falls prevention.
- An understanding of the 5 ways to wellbeing and how to maximise the impact for older people through the delivery of planned support activities
- Experience of one to one and group work to ensure clients meet support goals and achieve milestones they have set for themselves.
- Experience of handling and processing personal and sensitive data and the governance supporting these activities together with an understanding of GDPR
