**We are currently recruiting 2x full time positions. 1x 12 month fixed term vacancy in the planned care team in the Whitby locality. 1x permanent vacancy in the discharge hub supporting with local hospital discharges.**

As a Social Care Coordinator you will work operationally in Whitby, Scarborough and surrounding areas to implement the requirements of the Care Act with a focus on quality and best outcomes for local people. We can offer opportunities supporting people requiring mental capacity assessment, care act assessment, discharge to assess process supporting in a wide range of settings, community, supported living, extra care housing, hospitals, care homes, working with adults who are 18 plus and also with transitional cases.

Collaborative partnerships will be key to the role. You will work closely with various teams and agencies, including the NHS, our stronger communities and prevention teams and the voluntary and independent sector, promoting health and wellbeing, and preventing, reducing and delaying need. You must be flexible to support and respond to the changing needs of the service.

As a worker within the hub you will be working in a fast-paced environment, and will work closely with our health colleagues to ensure that seamless pathways of care are delivered under discharge to assess arrangements. You will undertake assessments under discharge to assess arrangements whilst being mindful of the importance of a strengths-based focus and preventative measures to ensure we have the person’s wellbeing at the heart of what we do. We can be creative with the approaches that we use to ensure that someone is able to have their needs met in a safe manner, whilst clearly noting the importance of thinking ‘Home First’.

You may need to be involved in considering the Mental Capacity of people being discharged from hospital, if this is called into question and requires Best Interests decisions to be made on their behalf involving both families, health professionals and other relevant partners in our wider communities.
