**Job Purpose**

To work collaboratively with the multi-disciplinary team across the PCN to increase capacity in nursing services.
**Job Summary**

Provide nursing associate services to patients in our PCN, helping us to decrease waiting times for appointments and give capacity back to our registered nurses to manage more complex health matters for our patients.
**Key Areas of Responsibility**
- Work collaboratively with colleagues across the PCN, supporting MDTs and registered nurses providing them with additional time to focus on more complex clinical care.
- Promotion of health and wellbeing to all patients by undertaking activities such as the NHS check.
- Provide care for individuals with dementia, mental health conditions and learning disabilities.
- Advise on general healthcare, self-management and personalised care, signposting, where appropriate, to relevant wider service across the locality.
- Provide and monitor care of patients working under direct or indirect supervision to include, where competent:

- Phlebotomy
- Blood pressure
- Pulse
- Respirations
- Weight
- Height
- Elimination pattern
- Eating pattern
- Sleep pattern
- Glucose pattern
- Support and supervise training nursing associates, healthcare assistants, apprentices, and others on placement, sharing your knowledge, skills and experience emphasising the importance not only of skills but working collaboratively and have a continuous improvement approach to safety and the quality of care.
- Your duties will complement and support the workforce across the PCN.
- You will work collaboratively across the PCN and wider including secondary care and mental health.
- Proactively communicate with colleagues including attendance and contribution in more formal settings/meetings.
- Ensure the patient records are accurate and kept up to date in line with relevant legislation.
- Ensure robust clinical governance is in place for your practice which includes regular audit and evaluation, supervision and CPD.

**Management of workload and personal development**

Act on own initiative in prioritising and managing workload, in respect of clinical, managerial, and administrative areas, as necessary.

Maintain own personal and professional development as agreed at your annual review.
