Loneliness and Dementia Friendly Communities
3 days ago
**Main Purpose of the role**
This blended role will offer support to some of our most vulnerable older people; seeking to promote healthy ageing and building resilience amongst our older communities.
Since the COVID pandemic, the local community sector has increased efforts to rebuild social opportunities for older people that were severely disrupted over an extended period. Loneliness and isolation behind closed doors is the resultant hidden pandemic that this role will seek to combat.
Alongside this aspect, Dementia is a disease (not a fact of later life) that requires a targeted form of support - helping both the person affected and their carers pre and post diagnosis.
Causes, effects and positive interventions for both the above are intrinsically linked, and so the work done here will often cross over depending on the context.
**Context and Collaborations**
The role is hosted by Age UK West Cumbria
The loneliness aspect is delivered as part of _Reconnecting Cumbria_, which is a Cumbria
The role supports the Allerdale Dementia Action Alliance (Covering Workington, Cockermouth, Maryport, Keswick & the Solway plain) In line with Local Government reorganisation, this role will cover the entirety of West Cumbria. (1 day per week)
**Key Tasks**
1. To instigate and encourage community groups, social activities and outreach that will support people to cope with and overcome isolation and loneliness in later life. In serving those who live with dementia and their carers, this includes Dementia Friends awareness sessions.
2. To identify and support people and their carers to access local community groups and activities in the area. To be aware of the emerging needs of clients and to work with partners to develop responses to meet these needs.
3. Find, engage and develop volunteers to support community outreach both as generalist community volunteers, befrienders and as dementia champions. To promote dementia champion training and keep a log of dementia champions to deliver dementia friends awareness sessions
4. To raise awareness about both loneliness and dementia in key organisations and businesses within the community that support people living with dementia and engage them in the “Dementia Friendly Community” initiative
5. To respond to requests for information and support from people with dementia and their carers in a friendly, courteous and professional manner.
6. To be aware of the range of services available for people in the area and to refer them appropriately.
7. To support the DAA to develop initiatives for people living with dementia and their carers, identifying gaps and providing social activities for people with dementia and their carers
8. To be aware of national initiatives focusing on loneliness and dementia
9. To work with partners to develop an information pack for people with dementia and their carers
10. To investigate the feasibility of the development of a “buddy phone group” (24 hours) - to provide support for carer’s who may need support and reassurance in dealing with their situation
11. To ensure the community is aware of the work of the Dementia Action Alliance by publicising activities through notice boards, local newspapers and social media
12. To organise, facilitate and record the DAA meetings. To complete and monitor DAA action log requesting information from members when necessary. To report to the Dementia Action Alliance at their meetings and prepare written reports.
13. To collate performance data for the purposes of monitoring and evaluation using pre existing impact measurement systems.
