An opportunity has arisen to work across an integrated dynamic, innovative and friendly team within Wirral Community NHS Foundation Trust Integrated Discharge Team (IDT).

The team is going through a very exciting period of transformation within the trust and an excellent opportunity has arisen for a position within the IDT team. We are looking for an enthusiastic and forward-thinking Care Navigator to join our busy, pro-active team based at Arrowe Park Hospital.

Please Note. This role will be Transferring to Wirral Council during early/mid 2023 as part of the whole Adult Social Care service. At the point of Transfer the role will TUPE to Wirral Council who will become your new employer.

The role will involve assessing and working with people with a wide variety of both physical and mental health conditions as well as complex social issues. This role will involve collaborative working and taking responsibility within our experienced multi-disciplinary teams - MDTs, to ensure best possible outcomes are achieved for people using the service, their carers and families. You will attend regular MDT meetings and ensure that you work effectively to provide person - centred care, ensure a timely journey for people who use the service, prevent discharge delays and avoidable admissions to hospital.
You need experience of working in a Health or Social Care environment role and have an good understanding of current social work legislative practice. There will be a need to ensure that you work in compliance of the requirements of The Care Act 2014. Risk management, excellent communication skills and an ability to prioritise and often work at a fast pace is essential. You will need good time management skills.
This post requires applicants to work alongside Social Workers in completing non-complex assessments, reviews of short-term placements to identify long term care needs, MCA and Best Interest Meetings where applicable. It is also essential that the post holder is able to be effectively resilient to challenges posed by the Acute Trust (hospital) during winter pressures.

The post holder will be required to actively participate in quality improvement initiatives and to ensure that they manage their continuous professional development. There will be the opportunity for regular supervision and to attend in-service training and professional development.
