Therapy Support Worker

The role requires day-to-day delivery of therapy in the Community, working alongside the Physiotherapists, Occupational Therapists and Advanced Therapy Support Workers in patients' own homes.
If successful, you will be offered a position based at Florence Nightingale Community Health Centre and you will be visiting patients in the community.
You will be working alongside the Community Therapists to provide rehabilitation for activities of daily living and mobility in patient's own homes.
We work closely with our Acute Hospital and Social Care colleagues to facilitate timely hospital discharges and work together to avoid unnecessary admission to acute care where able. together you make the difference.
Our integrated teams work together to provide a Therapy service which operates a shift pattern between 8.30am and 6.30pm, during the week and a shift pattern of between 9am and 5pm, at weekends and Bank Holidays.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
