**PURPOSE OF ROLE**:
To help support Carers with areas that are important to them including, employment / or lifelong learning, volunteering opportunities and community based activity.

To carry out What Matters conversations or Carer Needs Assessments in line with the Social Services and Wellbeing Act 2014.

To work with Carers and their communities on carer friendly initiatives that matter to the community, this may include Carer training, practical advice and support, signposting as required and ensure the Carer is best placed to explore interests and opportunities.

To raise the profile of Carers in the community, in local businesses and in local leisure and travel facilities etc. This may include supporting carers into paid employment, lifelong learning and volunteering.

**MAIN DUTIES AND RESPONSIBILITIES**:

- To identify carers who would like to explore opportunities that they feel will explore their wellbeing and ensure they are aware of the support available in the county.
- To facilitate What Matters Conversations with carers, working in partnership with Wrexham Local Authority.
- To work with carers to develop carer friendly initiatives, letting carers take the lead in their communities and develop projects that matter to them.
- Ensure the legislation affecting carers is cascaded to all.
- Develop working partnerships with relevant agencies to promote and support carers.
- Any Carer Friendly Initiative should be developed productively with Carers in Wrexham as equal partners.
- Provision of support to Carers and volunteers to access volunteering initiatives and opportunities that matter to the carer.
- Provision of support to carers when their caring role ends and support their inclusion into society through accessing employment and community involvement.
- To explore and support Carers to facilitate groups that they would like to develop or be a part of in their communities.
- Work with partners in the statutory and voluntary/community sectors to provide appropriate external representation of Carer issues, including BCU Local Health Board and Wrexham Local Authority.
- Maintain all NEWCIS monitoring procedures and contribute to day to day running of the NEWCIS Wrexham office
- Maintain and ensure confidentiality over personal information relating to individual users of NEWCIS and practice information.
- Undertake any other duties as reasonably requested by the management team.

**Job Type**: Part-time
Part-time hours: 22.5 per week

**Salary**: £22,502.00-£25,184.00 per year
