Dementia Identifier Wellbeing Officer

**PURPOSE OF ROLE**:
The Identifier support service will provide early identification of those diagnosed with dementia or other cognitive impairment outside the memory assessment services. The purpose of the role is to ensure the needs of those who are diagnosed with dementia or cognitive impairment are met as early as possible. This will be achieved by working with hospitals, Community Mental Health Teams (CMHT), and other relevant services.

**MAIN DUTIES AND RESPONSIBILITIES**:
The post holder will be the first point of call for those diagnosed within hospital or CMHT and other settings.

Attend ward and CMHT MDT meetings to identify and accept referrals from the teams for both the person with dementia and their carer/s.

Ensure consent has been gained by appropriate parties to share information with the project providers.

Ensure all consented information data is passed to the memory assessment tracker team and make appropriate referrals to the post-diagnosis support service.

Provision of one-to-one support will be to people diagnosed with dementia and their carers whilst in hospital and a referral passed to the post diagnosis support and dementia centres following discharge.

Initial support will be provided for those identified within the CMHT and other settings.

Provide one to one support, which may include appropriate assessments, to enable greater improvement in independence.

To seek and record client feedback to understand whether needs are being met, and to plan any necessary adjustments.

Develop a flexible approach to delivering services jointly in line with the social services and wellbeing act and other partners involved in the Memory Assessment Pathway.

To work in a manner that facilitates inclusion, particularly with the carers caring for people with dementia.

Ensure that all Carers can access a Carers Need Assessment via a non-statutory route, i.e. carer/cared for who are not in receipt of Social Services provision or who are not known to Social services.

To administrate and organise own work to ensure that it is accurate and meets quality targets, reasonable deadlines, and reporting requirements.

Maintain all NEWCIS monitoring procedures and contribute to day to day running of the service.

Maintain and ensure confidentiality over personal information relating to individual users of NEWCIS and practice information.

Undertake any other duties as reasonably requested by the management team.

**COMMUNICATIONS**:
Communication with carers is an essential part of the post to ensure good customer service is achieved, this includes updating the NEWCIS data base ensuring that relevant staff can access data in order to support the carer.

Work with the team leaders and staff in the co-ordination and production of communications, including digital newsletters, posters, advice literature, press releases and publicity materials as required by the post when facilitating services for carers.

Work with the administration team to co-ordinate the ordering and supply of communications and PR materials, liaising with the Administration Manager.

**GENERAL**

Teamwork is essential to the smooth running of the organisation and all members are required to play an active part. Staff duties include helping to maintain cover in the office, dealing with calls from carers and following all the policies and procedures of the organisation. Everyone has a responsibility to work co-operatively and in collaboration with all members to ensure that the organisation meets the requirements of the service level agreements it has entered into.

Due to the nature of the job and organisation, some evening and weekend work will be necessary for which time off in lieu can be taken.

**LOCATION**

The role is based at the Carer Centre in Wrexham and the Wrexham Maelor Hospital, however the jobholder may be required from time to time to work from the Mold office, as such a current UK driving licence and access to a vehicle or access to equivalent transport is required.
