Overview:
**Change Grow Live** are a charity dedicated to the belief that we can make a difference to our Service Users lives.

We have an exciting opportunity for two **Senior Practitioners within the Entry into Service and Opiate team **who have the skills and experience to assess and engage with our service users with complex needs and wrap around support. You will ensure that they access the right intervention at the right time in a way that best meets their needs.

You’ll have to record information about what you do, so enjoying a bit of paperwork and attention to detail is important. But most of the time, you’ll enjoy working with people from all backgrounds, making a difference in their lives. No two days are the same, and we treat every person who comes into our service as an individual, so being flexible, curious, and a good listener is key.

**Responsibilities**:
**About the role**:

- Support line manager with roles including supervision (with reflective practice), appraisal, performance and sickness management as well as ensuring staff follow CGL and external policies applicable to their roles
- Empowering staff to promote hope, empowerment, choices, control, and opportunities that assist service users reaching their full potential as individuals and community members.
- Support staff to improve service user outcomes through observed practice, providing timely values-based feedback
- You will have a small case load delivering wrap
- around intensive, trauma informed support to people with multiple and complex issues within their own independent tenancies in the community
- Regularly review the person centred support plan to update, record and monitor progress of caseload

**About you**:

- Have a strong understanding of substance misuse issues and experience of working within a similar field
- An understanding of the Housing First model and belief in the values and principles
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Promote the rights, responsibilities and informed choice of the people receiving the service, acting as an advocate to promote independence
- Ability to quickly build rapport; being persistent, determined and resilient
- A creative approach to problem solving; getting and keeping people engaged demonstrating empathy
