**Care Co-ordinator**

**Salary: £23,819 per annum (£12.38 per hour)**

**Contract: Permanent**

**Hours/days: Fulltime, 37 hours per week**

**Location: South Cerney**

We have an exciting opportunity for an enthusiastic **Care Coordinator** to join the Phoenix Health Group, where we focus on providing excellent personal care and a holistic approach to medical practice. Working as part of a Multi-Disciplinary Team you will support the GPs to identify and manage patients’ changing needs, ensuring that appropriate support is available to patients and their carers.

This is a dynamic and responsive role, which involves working with the Practice Team to support patient care, checking in on patients and following up on their wellbeing. We are looking for a good communicator, with empathetic listening skills, who has experience of working in a public facing role. You will provide administrative support for patient management, so must be IT literate.

We are a large Practice with around 24,500 registered patients. Our clinical team is supported by experienced patient care advisers and administrative staff. We are based at five sites in and around Cirencester and Tetbury. This post will be based at our South Cerney Surgery. Everyone working in the Practice is valued for the role they play in providing a high-quality service for our patients.
