**MAIN PURPOSE**:
This role is based at Osman House, a 16-bed unit in Scholes, Leeds which provides rehabilitation to people with an acquired brain injury. It is a unique community where the people we support can address their cognitive, behavioural, physical, and emotional difficulties.
We will train you to provide personalised support to the people we support. You will follow individual support plans to meet the needs and aspirations of each person. The focus is developing independent living skills: personal hygiene, meal preparation, exercise, leisure activities, socialising and personal care - the daily routine of regular life.
It is a really varied job, with all kinds of different activities to organise and participate in - both at Osman House and in the local Scholes community: art, crafts, music, sports, gardening, shopping, swimming, and countless others.

**MAIN TASKS AND RESPONSIBILITIES: (but not limited to)**
- During the course of your work you will at all times conduct yourself in a professional manner acting as a role model to colleagues and service users contributing to the delivery of quality services. To work as a member of the team, contributing to the team ethos and supporting service users(s) and colleagues appropriately.
- Facilitate the daily routines of the service users including personal care promoting a high standard of personal hygiene at all times.
- Participate in the development of service user(s) support plans, risk assessments and programmes, complete written records as necessary and in accordance with the guidelines provided.
- Implement individual support plans to meet the needs of the service user(s). Highlight any issues/concerns to the Team Leader to ensure the service user(s) receive quality services at all times.
