The Patient Pathway Co-Ordinator (PPC) will work within a wider team supporting the divisional services and specialties with all aspects of pathway management. Facilitating the smooth running of the patient pathway by ensuring that there are coordinated and streamlined administrative processes that revolve around the patient and their individual needs, proactively monitoring and tracking RTT, non-RTT, routine and urgent pathways to ensure that patient journeys are processed in line with nationally and local defined targets and that data is validated and inputted into local systems.

As this role forms part of a wider team of divisional administrative support, there will be an expectation of being able to flexibly work with various specialties as well as undertake a wide range of pathway related duties such as coordination and direct liaison with patients and various teams, booking appointments for outpatient activity, covering and supporting colleagues as appropriate.

To undertake the administration of all referrals received into the CAMHS teams, ensuring Patient Electronic Records are kept up to date

To act as an initial point of contact and deal with telephone enquiries from external agencies, service users and their relatives and other members of staff in a professional and sensitive manner, communicating messages where necessary, without delay to the appropriate individual.

To navigate patients through their care pathway from point of referral through to treatment in accordance with locally and nationally defined timescales using Patient Electronic records

To proactively identify and resolve delays at all stages of the patient pathway, using local stage of treatment targets in order to achieve Referral to treatment (RTT) targets. Where this is not possible, follow agreed process for escalation.

To attend meetings, and take minutes if requested, in an accurate and concise manner and to ensure they are typed, distributed, and stored, in accordance with group Terms of Reference and Trust policies and procedures.

Manage staff in defined administrative and secretarial groups.
