We're seeking a caring, responsible, empathetic, understanding and proactive individual with a great sense of humour to join our friendly **Scheduling/Care Co-Ordinator Team** at our Head Office in Hertford.

**Herts at Home **are a care provider working with Hertfordshire County Council and we provide care for people living in their own homes in the community.
- **Salary of £28,000 per annum**:

- You will work with a friendly, supportive team that share a passion for the delivery of high standard, person-centred care
- 5 Over 7 days working pattern, to include a Saturday or Sunday 1once a month. You will also be part of the contingency planning.

**The role and responsibilities**:

- Planning, coordinating and scheduling of services for our service users
- Using electronic scheduling system (People Planner) to organise rotas and ensure that all carer workers have their schedules in advance
- Produce accurate reports on scheduling and care planning activity to support data requirements for missed, late, and adjusted calls
- Delivering care as required
- Communication with brokerage services to discuss packages of care, increases/decreases.
- Act as a point of contact for relatives, carers, health and social care professionals in relation to scheduling and care packages
- Monitor alerts and escalate these as necessary to the Registered Manager

**About you**:

- Experience of customer assistance and call handling
- Experience of work-flowing planning and creating rotas
- Strong IT skills - knowledge of Excel and People Planner desirable
- Keeps calm and focused under pressurised and challenging circumstances
- Excellent communication skills
- A passion for delivering high quality care to a wide variety of older people
- Comfortable in assisting individuals with their personal care if required

Experience as a scheduler/planner/care co-ordinator would be an advantage but is not essential as full training is provided.
